Tirzepatide is a synthetic peptide that activates both the GIP receptor and the GLP-1 receptor.

FDA labeling describes it as based on the GIP sequence, containing aminoisobutyric acid at positions 2 and 13, a C-terminal amide, and a lysine at position 20 attached to 1,20-eicosanedioic acid through a linker.

The FDA-approved products, marketed as Mounjaro and Zepbound, are supplied as ready-to-use solutions in single-dose pens and single-dose vials — 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg or 15 mg in 0.5 mL — so their concentration is fixed by the label rather than by anything mixed by the user.

The same ingredient is also supplied as a lyophilized (freeze-dried) powder in compounded and research presentations; those are not FDA-approved products, and the concentration of one is set entirely by the mass in the vial and the volume of diluent added to it.
